Lawyer's note. An affidavit of loss must be subscribed and sworn before a notary public (jurat) to be accepted by agencies, banks and telcos. Describe the item precisely and the circumstances honestly — a false affidavit is perjury.

Frequently asked questions

How much does notarization cost?

Commonly a few hundred pesos per document, varying by city and notary. Call ahead — some agencies also have accredited notaries nearby.

What if I later find the lost item?

If a replacement was already issued, the found original is usually considered cancelled — surrender or destroy it to avoid confusion, and inform the issuer if required.
